MayaMD
Remote care management · Month 2 onward

The highest-risk days happen at home. We're there.

After the first 30 days, patients stay with your clinicians if appropriate via our tech-enabled service like remote patient monitoring (RPM), principal care management (PCM), advanced primary care management (APCM), and behavioral health integration (BHI), delivered through conversational AI, a companion app, connected devices and two-way text, with our medical assistants supporting your clinicians.

More programs, same team

Each program runs on the same engine, with the same staff supporting your clinicians, so patients never meet a stranger and your staff never juggles vendors.

RPM · FDA-CLEARED DEVICES

Remote patient monitoring

FDA-cleared cellular devices for blood pressure, weight, heart rate and oxygen, shipped to the patient. Readings reviewed daily against the care plan.

PCM · CPT 99424–99427

Principal care management

For specialty practices: monthly management of one complex chronic condition, such as kidney disease or heart failure.

BHI · CPT 99484

Behavioral health integration

Depression and anxiety screening, care planning and follow-up alongside medical care, because mental health drives readmissions too.

Questions practices ask

Which programs fit my practice?

Primary care practices usually lead with APCM and add RPM and BHI where they fit. Specialty practices often use PCM for one complex condition, plus RPM. We check eligibility and billing rules for each patient.

Do patients need devices?

Not for APCM, PCM or BHI. Devices are part of RPM, and we provide and support them.

What does my team do?

Review care plans and act on escalations. Our staff does the enrollment, outreach, documentation and claims prep.

Who qualifies?

Most Medicare patients who receive ongoing care from your practice. We screen your panel for you.

Is it compliant?

HIPAA compliant with a signed BAA, and workflows built around CMS program requirements.
