MayaMD
For patients and families

Going home from the hospital? You won't be on your own.

If your hospital or doctor works with MayaMD, you'll get a clear explanation of your care, before you leave or once you're home, then check-ins by app, text or phone, simple devices if you need them, and our staff working with your doctor for as long as you need it.

What to expect

Three stages. One team that knows your plan.

01 · DAY 0

Leaving the hospital

Before you leave or once you're home, Maya explains your diagnosis, medicines and warning signs in plain language, and checks that it makes sense to you.

02 · DAYS 1–30

The first 30 days

Our staff reaches you within two business days, by phone, text or app, on behalf of your doctor. You see your own clinician by day 7, and Maya checks in with you through the month.

03 · MONTH 2 ONWARD

Living at home

If your doctor recommends it, monthly check-ins, simple devices and support for your mood and stress help you stay well at home.

Your questions

What patients and families ask us.

Does it cost me anything?

Your doctor's team bills these services to Medicare like any other visit, and copays may apply. Our staff explains everything before you enroll.

Is a real person involved?

Yes. Maya checks in by app or text, and our staff follows up by phone on behalf of your doctor and lets your doctor's team know when something changes.

Is my information private?

Yes. MayaMD is HIPAA compliant and uses your information to support your care.

Not sure about a diagnosis?
